This form should be used to apply for an exemption to Article 166(4)(c) of the Air Navigation Order 2009 to allow a model to exceed 400 ft above the surface at a display or competition. CAP 658 should be read before submitting the form.

SRG 1725: Commission Regulation (EC) No. 2042/2003, Annex 1 Part M, Subpart G Compliance Checklist The Part M subpart G compliance checklist is designed to assist organisations applying for a Continuing Airworthiness Management Organisation approval to demonstrate how compliance with the applicable parts of the Part M regulation can be demonstrated. The completed compliance checklist should be provided to the assigned regional office surveyor during the initial assessment process. Completion of the checklist is not mandatory but will greatly assist the CAA with the initial assessment of the organisation's compliance and capability to hold this approval.

SRG1814: Application for Extended Operations (ETOPS) Operational Approval This form is used to apply for an approval to carry out Extended-Range Twin-Engine Operations (ETOPS). ETOPS applies to operators wishing to use twin-engined aircraft more than 60 minutes flying time from a suitable diversion aerodrome. Such routes include long ocean crossings, polar routes and trans-Siberian routes.
